KETOCONAZOLE HIGH DOSE HDK ; GUIDELINES Compiled by Charles Chuck ; Maack Prostate Cancer Advocate ; KETOCONAZOLE High Dose HDK ; guidelines accompanied by HC OR, SPECIAL NOTE - Dr. Strum now advocates ARISTOCORT TRIAMCINOLONE ; HOWEVER, I do not believe triamcinolone Aristocort is available in the U.S. ; to replace hydrocortisone as the accompaniment to HDK: "Now what I do is simply use triamcinolone at a dose of 2mg bid two times per day ; . I will decrease the dose if facial redness is severe or ankle edema. I have tried to use the 4mg bid dose but none of my patients has been able to tolerate it so far." HDK is initially prescribed at a dose of 200 mg three times a day for one week, then the dose is increased to 400 mg two tablets ; three times a day thereafter. HC is normally prescribed at a dose of 20 mg with breakfast and 10 or 20 mg with dinner. HC should be taken with food. If symptoms suggest HC excess ankle swelling or diabetes in poor control ; , the dose may need to be decreased. NOTE: Do not abruptly discontinue HC. Always discontinue HC by tapering the dose with the guidance of your physician. This may take several weeks. HOWEVER, PLEASE TAKE NOTE of Dr. Strum's change to triamcinolone at 2mg twice daily to replace hydrocortisone when treating with ketoconazole. You should discuss this with your physician ; . Moreover, histamine 2 H-2 ; receptor antagonists e.g. Zantac, Tagamet, Pepcid, Axid ; decrease HDK absorption by 75%. Proton-pump inhibitors Prilosec, Prevacid, Nexium ; reduce acid even more. Antacids and Carafate will also interfere with HDK bioavailability. Many other drugs have the potential to interfere with the absorption of HDK by their anticholinergic side effects that decrease stomach acid. Chest radiographs showed no abnormalities in 3 patients, diffuse reticulonodular infiltrates in 4 patients, and patchy areas of consolidation in 1 patient. On highresolution CT, 1 patient had diffuse micronodularity and patchy areas of mild ground-glass opacity, 3 had patchy areas of mild ground-glass opacity, 2 had patchy areas of both consolidation and ground-glass opacity, and 2 had bilateral honeycombing. Blood studies showed eosinophilia 15% ; in 1 patient, antinuclear antibody titer, 1: 320 ; in 1 patient, and precipitating antibody in 2 patients. A multiscreen test had positive results in 3 patients, and potatospecific IgE was detected in 2 patients. Six patients underwent bronchoalveolar lavage: Two patients showed more than 30% lymphocytes, 3 showed more than 25% eosinophils, and 1 had a normal cell population. Biopsy was done in 7 patients: Six specimens revealed nonspecific interstitial pneumonia, and 1 showed bronchiolitis obliterans organizing pneumonia. All patients had symptomatic, radiographic and functional improvement within weeks to months of leaving work, but none had recovered completely. Immunosuppressive therapy was used in 2 patients, and 1 patient required oxygen for 3 years after leaving work. Patients should "prime" or actuate ATROVENT HFA Inhalation Aerosol before using for the first time by releasing 2 test sprays into the air away from the face. In cases where the inhaler has not been used for more than 3 days, prime the inhaler again by releasing 2 test sprays into the air away from the face. HOW SUPPLIED ATROVENT HFA ipratropium bromide HFA ; Inhalation Aerosol is supplied in a 12.9 g pressurized stainless steel canister as a metered-dose inhaler with a white mouthpiece that has a clear, colorless sleeve and a green protective cap NDC 0597-0087-17 ; . The ATROVENT HFA Inhalation Aerosol canister is to be used only with the accompanying ATROVENT HFA Inhalation Aerosol mouthpiece. This mouthpiece should not be used with other aerosol medications.
